What a capsule contains and what reaches the bloodstream are different quantities, sometimes by a wide margin. The gap is created by a series of barriers between swallowing and circulation.
Dissolution comes first
A compound must dissolve in the contents of the gut before it can cross into the body, and many otherwise promising substances dissolve poorly.
Fat-soluble compounds dissolve into fat droplets, which is why some nutrients are absorbed several times better when taken with a meal containing fat than on an empty stomach.
Formulation addresses this directly through oils, emulsions and particle-size reduction, all of which aim at getting more of the material into solution.
Crossing the intestinal wall is selective
Some compounds diffuse across the gut lining, while others depend on specific transporters that have limited capacity and can be saturated.
Transport proteins in the intestinal wall also actively pump certain substances back out into the gut, reducing net uptake before anything reaches the blood.
This selectivity is why chemical form matters, and why two salts of the same mineral can differ substantially in how much is absorbed.
The liver processes the dose before circulation
Blood leaving the intestine travels first to the liver, where enzymes chemically modify many compounds for excretion.
This first-pass metabolism can remove a large fraction of an oral dose, which is the main reason some substances that work in laboratory studies fail to reach useful levels when swallowed.
It is also why an injected or sublingual form of the same compound can achieve very different blood levels from the same nominal amount.
Enhancers work by interfering with clearance
Some formulations pair a poorly absorbed compound with an additive that inhibits the enzymes or pumps responsible for removing it.
Raising blood levels that way is effective, and the same inhibition can slow the clearance of prescribed medicines processed by the same enzymes.
Interactions of this kind are a genuine clinical concern and are the reason a pharmacist should review supplements alongside any regular prescription.
Blood level is not the same as effect
Absorption is only the first requirement; the compound must also reach the tissue where it acts and remain there at a sufficient concentration.
Marketing that emphasises superior absorption is making a claim about one step in the chain, which does not by itself establish that anything downstream changes.
Evaluating a product therefore means asking whether an outcome was measured in people, not whether blood levels rose in a small comparison of two formulations.
